Post acute

At Glen Island Center for Nursing and Rehabilitation, our Post Acute care staff provides intensive, short-term skilled nursing and rehabilitative care. This highly interdisciplinary environment provides the opportunity for patients to enhance their independence and self-reliance. The goal is to restore patients to their maximum level of functioning, while treating them with the utmost compassion and dignity throughout the process. 
 
Each patient can benefit from our interdisciplinary care team, which includes a physician, nurse, dietitian, social worker, case manager, recreation personnel and a rehabilitative therapist. Other specialty consultants are available as necessary. Among the services they provide are: comprehensive medical, mental status and social assessments; ongoing medical and psychiatric support; and linkage to needed resources and healthcare professionals.   

Patients are admitted under several financial programs, including Medicare/Medicaid or private insurance.  A deputized Medicaid worker is on premise to provide information and/or assist in the Medicaid application process.
